Hammertoes are contracted toes usually on the second through the fifth toes.  They crunch up and end up causing pain, especially because they rub against the top of the shoe. Some of this pain can also be related back to the ball of your foot, and this is called retrograde pressure, which ends up causing too much pressure in the metatarsal head. Therefore, you are going to develop this metatarsalgia in this region too, and this is related to the hammertoes as well.
